I was in the process of replacing the water pump on a 03 2.4 2az-fe. It turns out the pump had been replaced before and the very bottom bolt was broken off. The pump never leaked, but now i have it off and am about to replace and have this issue. Not sure if I can a special right angle drill in there, don't think so.
